As a hospital case manager, my goal is always to ensure our patients are discharged home safely and receive the care they truly need and deserve, especially those transitioning to hospice care. Over the past few weeks, I've had the privilege of working with Jerry Martinez from Guiding Light Hospice on two of my hospice cases. Jerry has been exceptional in managing these transitions smoothly and compassionately. I also want to acknowledge Victoria, the clinical manager, who responded to my concerns with kindness and thorough attention to the treatment plans. We are truly grateful for the extraordinary care and compassion the Guiding Light team showed during my mom’s final hours. Nurse Perla was equally remarkable, guiding us through the process with clear explanations and ensuring we were fully prepared for what lay ahead. Their dedication and empathy made all the difference in making my mom’s last moments as peaceful as possible.
