About Hope & Grace Hospice

Hope & Grace Hospice was founded by former hospital case managers who identified gaps in care transition planning and built a hospice specifically to address those failures, bringing insider knowledge of hospital workflows and discharge processes. They excel at the intake process with thorough documentation and clear communication that discharge planners appreciate, understanding the regulatory pressures and quality metrics that influence referral decisions. Their founders' case management background means they think systematically about care coordination rather than just providing individual services, creating smoother transitions from hospital to home. Hope & Grace maintains detailed preference assessment during intake, reducing downstream problems from undocumented wishes and ensuring patient and family goals are clearly understood from day one. They provide discharge planners with same-day admission confirmation and thorough follow-up reporting, building a reputation as a reliable partner who understands the hospital perspective.

Current pt and warn against signing on. THE REVIEWERS LISTING QUALITY STAFF... NO ONE MENTIONED BY NAME IS THERE ANY LONGER! As you can see here they replied with private medical information on a public review. They only have 2 nurses both scared of service dogs, show up unannounced or reschedule and come for only minutes. Like less than 10. I've only met an aid at intake. I've never had a cna or aid visit. They don't help with pharmacieslike hospice Austin did. My previous no longer takes my visits but this is a bare bones, barely functioning hospice. I struggle to understand 2 of their employees and unlike other hospice they don't have true 24/7 care. Social worker doesn't make visits or seem to serve a functioning purpose (I'm a former sw) After hours staff is outsourced on the phone. They recommend ambulance knowing that will void hospice. I pray my insurance finds better care especially after the billing of services not provided. You'll get meds but nothing else here. Both nurses recommend going to er knowing patients lose hospice yet tell patients this will not happen. IF YOU VI TY THE HOSPITAL YOU WILL LOSE HOSPICE PLEASE CALL YOUR INSURANCE PROVIDER AND DON'T BELIEVE THIS. CHECK WHAT YOU'RE BEING BILLED FOR. I'M STUCK TEMPORARILY BUT THESE FOLKS ARE OVERWHELMED AND UNDERTRAINED. UNLESS YOUR LOVED ONES ARE WITHIN DAYS OF DYING GO ELSEWHERE. DON'T GET STUCK AND SUFFER AND STRUGGLE ALONE LIKE MY HUSBAND AND I. THIS IS AS BASIC AS IT GETS AND COMPARATIVELY THEIR SUPPLIES AND STAFF IS SUBPar. They are argumentative, don't show up and bill for services not supplied. A nurse rescheduled twice in one particularly sick and scary week. The replacement came barely made it up my stairs and then called while eating (self admission) "instructing" me to go to the ER and just later reapplying for hospice!!!!

My family and I cannot thank you enough to the staff of Hope and Grace. There are a few names that I would like to call out that work with my family: Jodi, Rose, Andrea and Natalie and of course the rest of the team that work behind the scene. Thank you for your kind attention to my father at the end of his life. We would not have done it without you guys help. The team was amazing, always there to support and we can call anytime for help. I would recommend this place to anyone that I know that needs the service and help for their loved one. Believe me; you will not regret to getting the service from this place to your loved one because they treat you and your loved one with the respect and the service that you deserved. To them, you like the friend and not just a number like any other big place does. Once again my family and I would always remember this place and the staff that we work with and always appreciate them for their hard work and delicate in their job. Thank you, The NGUYEN Family
