Heart to Heart Hospice's San Antonio Hospice House opened in October 2024 near the South Texas Medical Center — and is the only freestanding hospice house in the San Antonio area. Located on Huebner Road in a home-like setting, the facility serves patients from Bexar, Bandera, Gillespie, Kendall, Kerr, and Medina counties who need a higher level of care than can be provided at home. Private rooms with private bathrooms, common areas for dining and visiting, a children's area, and a chapel/meditation room make both patients and families feel genuinely welcome. The care team of physicians, nurses, hospice aides, social workers, chaplains, and bereavement specialists provides compassionate 24/7 care. Heart to Heart Hospice is CHAP-certified and has served the San Antonio community since 2009. Care is covered by Medicare, Medicaid, and most private insurance.

My wife fought Glioblastoma better than any warrior known. Once the Oncologist team informed us that there were not any further medical treatments available, they brought in a social worker to suggest hospice to us. Our social worker provided us with 8 different hospice companies to choose from. With my wife being the most important person in the world to me, her care matched that importance. My Mother-in-law and I painstakingly vetted each and every option provided to us, testimonials, services offered, website organization, and background information that could be sourced. We narrowed it down to two for an interview. Once Heart to Heart came to the house for the interview, their care specialist, Destiny, made it very clear that Heart to Heart was hands down the choice. Heart to Heart started caring with such love and compassion for my wife the moment we made our selection. From the care specialist, Destiny, to our nurse, Angela, our CNA Carolina, and even our on-call nurses Nicki and Melissa. Heart to Heart provided personalized, loving care every visit. Angela and Carolina became extended family and we were blessed to have them. The personal touch and the compassion provided was like nothing we had ever experienced. In such a delicate and crucial time in my wife's life these ladies were Angels on Earth to my wife and our family. Thank you, Heart to Heart, we brag about y'all daily.
Im very happy with Heart to Heart Hospice. Richard Dziuk has made all the difference. He met with my loved one and family. He was very informative and explained hospice care thoroughly. Sometimes hearing the process explained from a professional makes all the difference when loved ones are trying to figure out next step with a difficult transition. Heart to Heart Hospice provided exceptional care for my loved one during an incredibly difficult time. Although Richard Dziuk was not a nurse, he was incredibly knowledgeable and reassuring.
