About Heart To Heart Hospice - Austin
Heart To Heart Hospice Of Austin operates as a smaller provider that intentionally limits census growth to maintain personalized service and avoid the assembly-line feel families report from larger hospices, prioritizing quality over volume. They assign a consistent nursing team to each patient rather than rotating based on geographic routing, prioritizing relationship continuity over drive time efficiency so families build genuine trust with their nurses. The organization's smaller size enables the administrative team to know each patient's situation personally, not just as a medical record number, creating a more intimate care experience. When families call with concerns, they speak with staff members who recognize their name and can respond contextually rather than pulling up unfamiliar charts, making them feel truly known and valued. Their growth strategy emphasizes quality metrics and patient satisfaction over volume, accepting that staying small means limiting revenue but enables the relationship-based care that sets them apart.

If there were 10 stars I’d give them 11. My brother wasn’t on H to H Hospice care for long but, I’m grateful they took over & gave him a peaceful ending to this life on earth. Care, compassion, understanding meant everything to me. They were there when I needed it most. From my heart to y’all’s heart, I will never forget it, never. 💙
They helped both my mother and father . My mother passed a year before my dad did recently. A lady named Kirsten Crosby who works for heart to heart , was ther for my dad the last week's of his life , and we were very pleased with her , she was very compassion about the situation and caring and she was really the only one I know who could talk to him and explaine to him why what she was suggesting was the right thing to do , and get thru his head ...
Heart to Heart is an amazing company to work with. We’ve had the privilege of partnering with them on mutual clients, and every experience has been smooth and supportive. Their team works seamlessly with ours, making transitions easier for both clients and their families. A special thank‑you to Kristen C, (the social worker) & Susy Farris who are consistently going above and beyond to ensure our shared clients are cared for in every possible way. Her compassion and communication truly stand out. I highly recommend Heart to Heart Hospice.
